Intaking a Client

In order to enroll a client into OEWD-funded services and input information into WorkforceCentral, you must first complete the proper intake procedures.

  1. Meet with client (in-person or virtually) to assess their needs and discuss services.

  2. Collect signed documentation from the client.

  3. Gather signed documentation and input the information in WorkforceCentral.

  4. Maintain all documents (hard or electronic) in individual client case files.

Required Documents by Grant:

  1. Signed OEWD Workforce Development Application - all clients

  2. Signed Release of Information form - all clients

  3. Signed Participant Rights and Responsibilities - all clients

  1. Signed OEWD Master Application - all clients

  2. Signed Release of Information form - all clients

  3. Signed Participant Rights and Responsibilities - all clients

  4. WIOA Title 1 Eligibility Verification Form - WIOA Adult and Dislocated Worker only

  5. Signed Individual Employment Plan - WIOA Adult and Dislocated Worker only

  6. Signed Individual Service Plan - WIOA Youth only

  7. WIOA Youth Eligibility Checklist - WIOA Youth only

Best Practice/Tip: All intake steps must be followed for data entry into WorkforceCentral, but you may allow for time between steps to either allow a client to retrieve information or for you to be certain that client is ready to be enrolled in services.
